Consent to the Processing of Personal Data
I hereby consent to the processing of my personal data in the form of: first and last name; street; house number; apartment number; postal code; city; country; contact phone number; e-mail address; company name; tax identification number (NIP), for the purpose of providing services to me by the Controller MEGAL Sp. z o.o., 04-202 Warsaw, ul. Marsa 1, NIP: 532-17-90-965, REGON: 017447883.
I declare that, pursuant to Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 (General Data Protection Regulation - GDPR), I have been informed that:
- the controller of my personal data is MEGAL Sp. z o.o., 04-202 Warsaw, ul. Marsa 1, NIP: 532-17-90-965, REGON: 017447883,
- my personal data will be processed solely for the purposes indicated above and may be transferred to third parties, with recipients being entities providing services to the Controller on the basis of data processing agreements,
- my personal data are processed for the purpose of performing the contract concluded between me and the Controller and, on the basis of my consent, for direct marketing purposes,
- my personal data will be stored for the duration of the contract concluded between me and the Controller,
- I have the right to request from the Controller access to my personal data, rectification, erasure or restriction of processing, the right to object to processing, and the right to data portability,
- I have the right to withdraw my consent to the processing of my personal data at any time without affecting the lawfulness of processing carried out on the basis of consent before its withdrawal,
- I have the right to lodge a complaint with a supervisory authority,
- providing personal data is voluntary; however, failure to provide such data will result in the inability to perform the contract concluded with the Controller,
- the Controller collects data in an automated manner but does not apply profiling to me.
- I declare that I am of legal age and have full legal capacity. I also declare that I have read the entire content of the above consent and fully understand it.
